Not only are SIF events devastating, they are also notoriously difficult to prevent: Traditional safety metrics do not detect exposure to SIF risk.

Why is it important?

The first reason is simple: Saving more lives.

The second reason is not as simple: To get leadership to “see” exposures that both resulted in SIFs and have the potential to result in SIFs.

That seems straightforward but it’s not. According to the Occupational Safety and Health Administration (OSHA), employers reported 4,185 fatalities and 23,282 severe injuries between January 2015 through April 2017. However, OSHA estimates that perhaps 50 percent or more of severe injuries have gone unreported! As a result, OSHA has issued $5.2 million in penalties for failing to report SIFs.
